ABSTRACT

【Objective】 To explore the effects of status of lymph vascular invasion (LVI) on the survival of patients with squamous cell carcinoma of the penis (SCCP). 【Methods】 Data of patients diagnosed as SCCP during Jan.1, 2010 and Dec.31, 2019 were extracted from the Surveillance, Epidemiology, and End Results (SEER) database. Kaplan-Meier method was used to draw the survival curve of patients with different LVI statuses, and log-rank test was conducted in parallel. Univariate and multivariate Cox regression analyses were used to assess the effects of LVI status on the overall survival. Patients were divided into different subgroups based on stage (localized, regional, and distant metastasis) and grade (well, moderately and poorly differentiated) of tumor, and the effects of LVI status on the overall survival of patients in different subgroups were assessed. 【Results】 A total of 1 435 patients were involved, including 1 102 (76.8%) without LVI and 333 (23.2%) with LVI. Median survival time of patients without LVI and with LVI were 27.5 months and 17.0 months, respectively (χ2=55.028, P<0.001). Cox regression analyses showed LVI was a significant prognostic factor in SCCP patients (HR=1.280, 95%CI:1.044-1.569, P=0.018). In the subgroup analysis, LVI was an independent risk factor affecting the overall survival of patients with localized tumor (HR=1.446, 95%CI:1.009-2.110, P=0.046) and regional tumor (HR=1.323, 95%CI:1.018-1.720, P=0.036);it was also an independent risk factor affecting the overall survival of SCCP patients with well differentiated tumor (HR=2.797, 95%CI:1.573-4.971, P=0.046) and moderately differentiated tumor (HR=1.431, 95%CI:1.071-1.914, P=0.015). 【Conclusion】 LVI status is a significant factor affecting the prognosis of SCCP patients. LVI is an independent risk factor for the overall survival of SCCP patients with localized and regional tumor, moderately differentiated and well differentiated tumor.

ABSTRACT

Objective To observe the effect of Yiaikang capsules on viral load, immunologic function and quality of life of patients with human immunodeficiency virus infection and acquired immune deficiency syndrome (HIV/AIDS). Methods A prospective randomized controlled clinical study was conducted, 118 patients with HIV/AIDS admitted to Department of Integrated Traditional Chinese and Western Medicine of Qinghai Fourth People's Hospital from July 2015 to February 2017 were enrolled, and they were divided into two groups by random digital table method, 59 cases in each group. The control group received highly active anti-retroviral therapy (HAART); while the treatment in combined Chinese and western medicine group was additionally given Yiaikang capsules on the basis of the therapy in control group, 5 capsules (0.5 g per grain,) once and 3 times a day for 12 months. The differences of World Health Organization HIV quality of life scale (WHOQOL HIV-BREF) score, CD4+, CD8+T-lymphocyte and viral load levels were compared between the two groups. Results After treatment for 12 months, the results of WHOQOL HIV-BREF scores in two groups were reduced significantly compared with those before treatment (P < 0.05), and the degree of decrease of WHOQOL HIV-BREF score in combined Chinese and western medicine group was slower than that in control group (82.57±8.76 vs. 70.53±9.45, P < 0.05). The CD4+, CD8+levels in control group after treatment were of no significant change compared with those before treatment (P > 0.05), but the viral load level was decreased significantly after treatment compared with that before treatment (log/mL: 3.57±0.82 vs. 3.89±1.32, P < 0.05); the CD4+in combined Chinese and western medicine group after treatment was obviously higher compared with that before treatment (number/μL: 413.67±187.39 vs. 376.65±206.51), the viral load level was markedly lowered compared with that before treatment (log/mL: 3.14±0.76 vs. 3.81±1.27) and the level of CD4+was significantly higher in combined Chinese and western medicine group than that in control group (413.67±187.39 vs. 382.72±194.82), viral load level in combined Chinese and western medicine group was siginificantly lower that in the control group (3.14±0.76 vs. 3.57±0.82, P < 0.05), but the number of CD8+in two groups after treatment was of no significant change (P > 0.05). Conclusion Yiaikang capsules possibly may elevate the number of CD4+T-lymphocytes and decrease the level of viral load to improve the quality of life in patients with HIV/AIDS.

ABSTRACT

BACKGROUND: Stroke in progression(SIP) accounts for 26% -43% of cerebral infarction. It is a common clinical stroke subtype. However, there are no effective treatments to stop the progress with higher mortality rate and invalid rate. More attentions have been paid for multiple risk factors of cerebrovascular diseases during precaution and treatment. But the study on relationship between infection and stroke occurrence, and the inhibited clinical event with antivirus druggery is just at the beginning.OBJECTIVE: To investigate the correlation between clinical SIP characteristics and herpes virus(HSV) infection, and provide the clues to the SIP diagnosis and treatment and early rehabilitation.DESIGN: Case-control study based on patients.SETTING: Department of neurology in a university hospital and a microbiological department in a university.PARTICIPANTS: Totally 47 patients with progressive stroke hospitalized in Department of Neurology, the Second Hospital of Hebei Medical University during April 2001 to March 2002[21 males, 26 females; aged from 15 to 78 years, mean age of(53.12 ± 2.45) years] . There were 43 cases of limb paralysis (92%), 7 cases of aphasia, 4 cases of consciousness disturbance, 1 case of epileptic seizure, 1 case of hemianopia, 5 cases of ataxia, and 24 cases of dizziness. The diagnosis of cerebral infarction was confirmed with CT and/or MRI. The patients clinical pregresively aggravated over more than 72 hours from the stroke onset. And it was not able to stop the aggravation of the situation with conventional treatment. The patients with embolism and reversible ischemic neurological deficit(RIND), cerebral hemorrhage, hemorrhagic infarct were excluded; Patients with other severe diseases such as coronary heart disease, cardiac atrium fibrillation, severe diabetes, hypertension, and the history of illness more than 5 years were excluded. Totally 193 patients were selected as control group with non-cerebrovascular disease, included headache, insomnia, neck and shoulder pain[ 100 males, 93 females, aged from 26 to 60 years,mean age of(42.23 ±9.29)].of the Fourth Scholarship Meeting of Cerebrovascular Diseases in China in tected by MRI, 44 cases detected by magnetic resonance angiography(MRA), 4cases detected by digital subtraction angiography (DSA), and 8 cases detected levels of IgM of human cytomegalovirus(HCMV) and herpes virus(HSV) in serum of 47 patients with progressive stroke and 193 patients without cerebrovascular disease were detected by dot immunogold labeling staining (IGLS); Stromatin PP65 antigen of HCMV in marrow cell were detecting by of the situation with the conventional treatment. PFA 250 mL(3.0 g) was given once a day from 14 to 21 days and dexamethasone 5 - 10 mga day for 3 to 7 days.after treatment with antivirus druggery.dot low-density plaques in cortex or subcortex of cerebral lobes, were found in were found positive in 91% of the patients and the level of IgM in patients PFA was effective.of cerebral lobes, and stenosis or occlusion in cerebral arteries or major infection and progressive stroke.
